Social Work Choice UK Agency are recruiting for a local authority looking for an exciting opening for a care coordinator. This role is for a care coordinator, on scale 4 point 7. to work with adults service Outreach. The role of this worker would be to monitor support visits are logged and developing staff rota's using our call monitoring system. previously known as CM2000. This worker will have responsibilities to liaise with staff send out rota's and monitor the visits are being completed. monitoring of our team email and responding to referrals coming through, logging them on to a spreadsheet and tracking with team leaders. 18 hours weekly on a rolling rota basis. rota is:


Week 1 - Tuesday 9-5pm, Wednesday 9-4.30pm and Thursday 9-1.30pm.

Week 2 - Monday 9-12.45pm, Thursday 12-5pm and Friday 9-1pm

Duties & Responsibilities


  • Support the day-to-day financial, clerical and administration functions

  • Maintain effective and efficient administrative and information management systems

  • To attend and provide administrative support

  • Use relevant databases, in the course of normal duties, to include data input, data interrogation as directed

  • Deal with straightforward customer enquiries and follow through to resolution

  • Ensure data and records are maintained in accordance with agreed standards

  • Assist with the provision of an accessible customer care focused reception service

  • Provide a front line face-to-face/telephone service to customers

  • Identify any difficulties visitors/service users may have in making an enquiry or receiving information

  • Attend regular service meetings and, with colleagues, contribute ideas and make recommendations

  • To develop and maintain a working knowledge of relevant policies, procedures and legislation

  • To work as part of a project team and to assist in the delivery and completion of work projects

  • Advise and train colleagues in any specialist activities undertaken / areas of expertise

  • Provide support to colleagues and resolve queries relating to corporate and department specific systems

  • Responsible for the handling of service users’ monies, where required.

  • Use the Council’s electronic procurement system (SRM) for the provision of goods and services

  • Assist managers with day to day and year-end financial processes, where required.

  • Attend service specific meetings, which are sometimes of a confidential and sensitive nature

  • Within the scope of general guidelines, resolve and choose courses of action for problems that are not straightforward and require discretion

  • Assist less experienced colleagues to efficiently resolve more complex enquiries to successful completion

  • Organise and prioritise own workload as appropriate

  • Assist managers in carrying out Health and Safety checks/audits

  • Manage own performance effectively to meet goals and targets and work to required deadlines

  • As discussed with the line manager, take responsibility for own learning and development and participate in performance management and development discussions in line with the performance appraisal review process

  • To undertake any other reasonable duties commensurate with the grading of the post

  • At all times to carry out responsibilities with due regard to the Council’s Code of Conduct and related policies


Qualification & Experience

* NVQ/QCF or equivalent Level 2 in Social Care or IT

* Experience of working in an adult social care environment

* Experience of working with an electronic rota allocation system

* Competent when using the IT systems
